How can you access an ADP authorizer?

If you are a resident of a nursing facility or a home for the aged, ask the nursing staff and they will refer you to a Community Care Therapist or contact 2Care4 Medical Ltd. directly and we can refer you as well.

If you are a patient in a hospital, speak to your doctor and they can refer you to an Occupational Therapist or a Physiotherapist to assist you.

If you live in the community, you have 2 options to acquire an assessment and your options are:

You may contact the Community Care Access Centre in your area and ask for Referral for a Physiotherapy or Occupational Therapist assessment.

You may also choose to go the private Occupational Therapist or a private Physiotherapy Therapist route for which there will be a fee.

If you are presently registered with Community Care Access Centre, simply ask your coordinator or case manager about the program and they will assist you in making arrangements.
